Fill in the booking form to book a UFV library video or DVD for a class show date. If you have multiple requests for titles throughout the semester, you may attach a list below.

Please allow 3 business days to process your request.

If you need to request a video from another institution, please use our Interlibrary Loan Request: Video/DVD form.
